CATASTROPHIC CARE MANAGEMENT Medical Case Management Group (MCMG) Catastrophic Case Management Services is an intricate, customized Plan of Care for patients with complex diagnoses, often with critical health issues as well as multiple co-morbities. MCMG has designated Catastrophic Case Managers who specialize in developing and executing in-depth treatment plans for Catastrophic Claimants.
The Treatment Plan is designed with the following goals: ·Assist the patient and family to secure optimal care and achieve full recovery. ·Address efficient delivery of Medical Services from the physicians, hospitals, staff and ancillary services, which include (but are not limited to) Transplant Team
·Secure the needed medical/surgical services with cost effective solutions. ·Improve the quality of life with the overall goal of return to pre-injury status ·Expert Case Management intervention insuring the client’s satisfaction with their case progress while allowing less costly medical decisions to be made ·Consistent, “Team”/patient communication and integrated healthcare management. ·Patients who meet the Catastrophic Case Management definition may have any one or a mix of the following (including, but not limited to): Acute Multi Trauma ·Cost of services is determined on a case by case basis.
